Is there a codified text (similar to Shulcan Aruch) specifically for Sephardic customs and Halacha explanations? If so, where can one get it?

The Shulchan Aruch is exactly that. The author of the Shulchan Aruch was Sephardic and he discusses what the Sephardic customs are.
