Does my glass kitchen table top require tvilah?

The non Jew would be required to lift the table so as to make a ‘kinyan’. Just telling yourself that you are gifting the table doesn’t work. And even just telling the non Jew that you are gifting it doesn’t help. You need the non Jew to lift the item or table for the gifting to take effect.

Would a heavy table be considered mechubar lekarka and thus not a keli?

Still requires tvilah

If you always cover it with a table cloth does it still require tevila?

No, but you still wont be able to place food directly on the glass table top without a table cloth.
